In More > Admin > Configure > Consent Forms, you can build custom consent form templates using basic HTML, text, and system variables. From this page, you can also edit and save changes to existing consent form templates.
Consent form templates cannot be deleted.
Once created, custom consent form cannot be deleted. You can disable a consent form by toggling off the Enabled option for the form in your Admin settings.
How does a client sign a consent form?
When you open a new consent form in Patient Chart, the Vetspire system automatically displays a signature box where the client can provide their signature on a desktop computer or tablet. Select Finish after they have drawn their signature. You can also send the consent form to the client via email or SMS (text) message to be signed prior to their appointment or at a later time.
