Forms tab
The Forms tab lists all signed and unsigned consent forms associated with the patient and the status of each form. Below are the tasks available from the Forms tab.
View or capture a new consent from the Forms tab.
Creating a custom form requires HTML coding More > Admin > Configure > Consent Forms. For help, please open a ticket through support.vetspire.com.
View Consent Form
See a list of all the Consent Forms for a patient, the form's Name, the Status, the Patient the form pertains to, the Date the form was generated, and the Location the form was generated from.
To access consent forms, select the Forms tab on the patient screen.
Select the form from the list to see the form.
